Mesothelioma Stages



MESOTHELIOMA STAGES
Treatment options are often determined by the stage of mesothelioma a patient is in. There are three staging systems currently in use for pleural mesothelioma and each one measures somewhat different variables; peritoneal mesothelioma is not staged.
The oldest staging system and the one most often used is the Butchart System which is based mainly on the extent of primary tumor mass and divides mesotheliomas into four stages. The more recent TNM system considers variables of tumor in mass and spread, lymph node involvement, and metastasis. The Brigham System is the latest system and stages mesothelioma according to resectability (the ability to surgically remove) and lymph node involvement.
Butchart System – extent of primary tumor mass
Stage I: Mesothelioma is present in the right or left pleura and may also involve the diaphragm on the same side.
Stage II: Mesothelioma invades the chest wall or involves the esophagus, heart, or pleura on both sides. Lymph nodes in the chest may also be involved.
Stage III: Mesothelioma has penetrated through the diaphragm into the lining of the abdominal cavity or peritoneum. Lymph nodes beyond those in the chest may also be involved.
Stage IV: There is evidence of metastasis or spread through the bloodstream to other organs.
TNM System -- variables of T (tumor), N (lymph nodes), M (metastasis)
Stage I: Mesothelioma involves right or left pleura and may also have spread to the lung, pericardium, or diaphragm on the same side. Lymph nodes are not involved.
Stage II: Mesothelioma has spread from the pleura on one side to nearby lymph nodes next to the lung on the same side. It may also have spread into the lung, pericardium, or diaphragm on the same side.
Stage III: Mesothelioma is now in the chest wall, muscle, ribs, heart, esophagus, or other organs in the chest on the same side with or without spread to lymph nodes on the same side as the primary tumor.
Stage IV: Mesothelioma has spread into the lymph nodes in the chest on the side opposite the primary tumor, or extends to the pleura or lung on the opposite side, or directly extends into organs in the abdominal cavity or neck. Any distant metastases is included in this stage.

Chemotherapy, Its not a Cancer Cure
This year marks the 60th anniversary of the first report of a trial for chemotherapy to treat cancer. Do you know that it evolved from the use of poison gas during both the world wars. Gas warfare killed 91,000 soldiers, and invalided another 1.25 million, in World War I, including one sulphur mustard attack in 1917, which claimed 14,278 casualties in very short time. And so the use of sulphur mustard continued, by the Italians against the Ethiopians in 1936, and in an accidental explosion at Bari during World War II, which consigned a thousand soldiers to a slow and agonising death. After the Bari attack, doctors noticed that the victims’ white blood cell count dropped alarmingly, and wondered whether nitrogen mustard might have some medical use. Cancer starts with the mutation of the genetic materials in the chromosome of the affected cell. The key is to reverse this genetic damage. Cancer results not only from the unabated genetic damage, but also from the failure of the prevention of repair systems of the body that are meant to stop cancer cells from multiplying, and spreading.

Who is at Risk for Mesothelioma
Mesothelioma, the deadly disease which has topped news headlines for the last decade, is contracted through exposure to airborne fibers of asbestos. It can be safely said that almost everyone in the world has been exposed to asbestos in varying degrees. But this sort of statement perhaps oversteps the bounds of rationality. While it is certainly true that individuals who have been exposed to small amounts of asbestos for relatively short periods of time have contracted the disease, this is a rarity rather than a regularity. In fact, the vast majority of Mesothelioma cases involve individuals who were exposed to airborne asbestos fibers for intense and extended periods of time.

There are, however, some other minor factors which can affect the likelihood of contracting the disease. Among these is radiation exposure. There have been causes in which individuals who were exposed to radiation have subsequently displayed signs of and ultimately contracted mesothelioma. However, empirical studies to attempt to verify this correlation have not proven out this claim very strongly.
Another more certain cause of the disease was the taking of the Polio vaccine between 1955 and 1963. Some batches of this vaccine were contaminated with Simian Virus 40 which has been detected in a host of rare cancers including Mesothelioma.
Those involved in construction or who lived in homes in the Cappadocian region of Turkey are also particularly at risk. In this region, homebuilders used Erionite, which is a type of zeolite silica stone. Exposure to the fibrous strands of this building material have led to an annual death rate of roughly 1% of the population which dies of mesothelioma each year in Turkey.
